Around here (New Hampshire) historical markers are cast metal roadside signs on a signpost. They may commemorate a building, but it also could be an event or a natural formation or the site of a former building or just about anything. The most common thing on a building is just a sign with the year it was built, although you occasionally get plaques with about the same content as a blue plaque. I don't think there's any standard format though.
There's no standard signage associated with the U.S. National Register of Historic Places, as far as I know.
